Our office is licensed in Illinois to complete drug/alcohol evaluations for all driver’s license reinstatement hearings with the Secretary of State, regardless of which county the DUI took place in the past. COURT-ORDERED EVALUATIONS AND DUI SERVICES

Most courthouses in Illinois allow any SUPR-licensed provider to conduct and submit a drug/alcohol evaluation to their court. However, some courthouses have "restricted evaluation providers," such as Cook, Dupage, and Lake Counties. Please review our service areas for specific information on which counties we provide services. The below listed pages list our services in restricted courthouses:

APPROVED PROVIDER LISTS

Some courthouses restrict or refer DUI services by using an "Approved Provider List." In these counties, courthouses approve certain local agencies for drug/alcohol evaluations, DUI risk education, and substance use treatment. Cloudgate Healthcare is an approved provider in the following courthouses:

COURT EVALUATIONS AND DUI TREATMENT

​

Many Illinois courts do not use restricted evaluators or approved providers. In these counties, it is generally required that a DUI provider have SUPR-licensing to perform a DUI intervention or conduct treatment.

 

Our facility is SUPR-licensed to perform alcohol/drug evaluations, DUI risk education, and substance abuse treatment, including treatment for DUI court cases. We provide online DUI Risk Education and DUI treatment in several counties. Among our service areas, we complete DUI evaluations and treatment for court in many counties, including the following:
