ONLINE DUI EVALUATIONS, RISK EDUCATION, AND TREATMENT

Cloudgate Healthcare offers both virtual and in-person services. Our physical office is in downtown Naperville, Illinois. Our office offers free public parking and is in the heart of downtown Naperville’s shopping and restaurant district. 

Our facility also embraces virtual care. We believe that telehealth provides clients with a convenient way to access our services. All of our DUI classes can be completed online. We regularly perform virtual evaluations, DUI Risk Education, and online alcohol/drug treatment for DUI cases in a HIPAA-compliant Zoom forum. 


EVENING AND WEEKEND HOURS

Cloudgate Healthcare understands that many of our clients have a busy lifestyle, and we try to accommodate a wide range of schedules. We provide evening and weekend treatment sessions in a virtual setting. Clients can log on to our secure Zoom platform to complete an evaluation or treatment session from their home or workplace. Our facility also accommodates virtual evaluations, DUI Risk Education classes, and individual treatment sessions by appointment. 


COURT SERVICES: DUI EVALUATIONS, RISK EDUCATION, AND TREATMENT

 

Evaluations for Court Purposes

 

The Illinois Secretary of State will accept a drug/alcohol evaluation from any licensed provider at a driver’s license reinstatement hearing. However, some courthouses mandate that the defendant obtain an evaluation from a specific courthouse provider in a pending DUI case.

Some counties have specific evaluation agencies to satisfy the court in a pending DUI case, such as Cook, Lake, and Dupage Counties. In contrast, Will County allows any approved provider to complete an evaluation for a DUI defendant. If a DUI defendant is not certain if they can use a private agency, please review our service area or contact our office at (630) 780-0262. ​

Court-Approved DUI Treatment Services

 

Although some courts require their own evaluation agency to complete the evaluation, a client may attend treatment at any approved facility. Our outpatient facility is on the approved DUI treatment provider list for most restricted courthouses, including Cook, Dupage, and Will Counties. We are licensed statewide through the Division of Substance Use, Prevention, and Recovery, a subdivision of the Illinois Department of Public Health. ​Our facility also offers educational services, such as Early Intervention and DUI Risk Education. Risk Education is a ten-hour course that is required to complete the recommended intervention for Minimum, Moderate, and Significant Risk classifications. Our facility is licensed to provide DUI Risk Education for court and/or Secretary of State driver’s license hearings.

Evaluations for Secretary of State Hearings

The Illinois Secretary of State will accept drug/alcohol evaluation from any substance abuse facility that is licensed in Illinois. As an Illinois-licensed substance abuse facility, Cloudgate Healthcare is knowledgeable about evaluation and treatment assessment requirements for Illinois Secretary of State hearings.

Our licensed clinicians gather all information from each client to draft clear and individualized documents for submission at Illinois driver’s license hearings. Our Secretary of State evaluations include a detailed chronological use history, as well as a comprehensive treatment needs assessment. We do not charge extra for these items as they are required when completing a new Uniform Report.

If a client requires additional services, we inform them of any added cost prior to scheduling the appointment. Our facility provides one transparent, upfront flat rate for a Secretary of State evaluation.

Treatment for Submission at an Illinois Driver’s License Hearings after a DUI Revocation

​

Cloudgate Healthcare regularly completes treatment for revoked drivers who require our clinical services for a Secretary of State hearing. We are familiar with the Secretary of State document standards for an alcohol or drug-based hearing.

At the conclusion of treatment, we prepare the following items for every Secretary of State client: Secretary of State Treatment Verification Form, Individual Treatment Plan, and Discharge Summary. If continuing care was performed, every client is provided with a Continuing Care Plan and Continuing Care Status Report. If continuing care was waived, we write a detailed waiver stating our rationale for our waiver.

We strive to make treatment accessible, affordable, and positive. We provide a fee estimate during our initial call with every client. Our fees are competitive, and we accept all major credit cards. Clients may pay as they go throughout our process to help with budgeting.

Response to Denial Order from Prior Hearing

​

If a client has been denied at a prior hearing, we regularly write “Response to Denial” letters for Secretary of State hearings. A client should provide a written copy of their denial letter to our office for review at the time of evaluation. During the evaluation, we discuss the concerns raised in the denial letter with the client to determine our response. Once our response letter is completed, we provide the client with our written response for submission at their next hearing.


ATTORNEY REVIEW

We recognize that many clients are working with a defense attorney for this DUI court case, or a driver’s license reinstatement lawyer for their Secretary of State hearing. If a client signs a written release, we are happy to forward the evaluation and treatment documents to their attorney upon request. 


EMERGENCY SERVICES

​​

As an outpatient substance abuse facility, we provide scheduled evaluations, DUI risk education courses, and alcohol/drug treatment sessions. If a client is encountering a medical emergency, we encourage them to contact 911 or go to their nearest hospital for care.
