ONLINE DUI TREATMENT AND RISK EDUCATION IN COOK COUNTY

Cloudgate Healthcare is an approved DUI treatment provider for individuals with a DUI case in Cook County, Illinois. Our facility is authorized by the court to provide the following outpatient intervention services:

COOK COUNTY COURT REQUIREMENTS AND APPROVED PROVIDER STATUS
1. Cook County DUI Evaluations for Court. Cook County DUI defendants are required to obtain their initial DUI evaluation through the court's designated evaluation agency. To schedule a DUI evaluation for court in Cook County, DUI defendants may request an appointment by email by sending a message to SSD.DUIeval@cookcountyil.gov. An appointment can also be made for a court evaluation by calling (312) 948-6001.
By law, a defendant should complete their DUI evaluation before sentencing. The evaluation is used by the court to determine the appropriate level of intervention for sentencing. Once the court evaluation is complete, the court permits clients to select any provider listed on the Cook County Approved Provider List for the intervention required by the evaluation.
2. Cook County DUI Risk Education and Treatment Services for Court. Cloudgate Healthcare is authorized to complete the required intervention identified in the court evaluation. This includes Risk Education for minimum, moderate, and significant risk clients, as well as outpatient substance use treatment for individuals classified as significant or high risk. We offer in-person sessions, as well as online DUI Risk Education and DUI treatment.
3. Secretary of State Evaluations, DUI Risk Education, and Alcohol/Drug Treatment. Cloudgate Healthcare is licensed by SUPR to provide clinical services for individuals attending Secretary of State license reinstatement hearings. Clinical licensing for Secretary of State hearings is determined by SUPR licensing, not the Cook County Provider list.
THE CLOUDGATE PROCESS
1. Initial Contact. Clients begin the process by calling our office or submitting a secure contact form. Our team answers questions and schedules the intake appointment. Daytime, evening, and weekend appointments are available.
2. Evaluation.

Court Cases: After completing the Cook County court evaluation, the client provides a copy of the report to Cloudgate Healthcare. This report identifies the assigned risk level and required intervention. Our agency is licensed to provide education or treatment to all outpatient risk levels (minimum, moderate, significant, and high risk).

Secretary of State Hearings: Clients pursuing driver’s license reinstatement can complete a drug/alcohol evaluation with Cloudgate Healthcare. This includes reviewing the current Court Purposes Driving Abstract and any DUI-related documents, including sworn reports. Our office completes a detailed chronological use history and treatment needs assessment for every Secretary of State evaluation to comply with their standards.
3. Scheduling Services. Based on risk classification, services may include Risk Education, early intervention, outpatient treatment, or a combination. Most services can be scheduled around the client’s availability, with options for in-person or online DUI classes.
4. Clinical Support. Each client receives individualized clinical support. Group and individual sessions focus on substance use education, personal insight, legal consequences, health risks of substance use, prevention skills, and development of positive leisure pursuits. Progress is monitored and documented by our agency.
5. Documentation. Upon program completion, clients receive original copies of their documents. These may include a alcohol/drug evaluation, Risk Education certificate, treatment records, and continuing care reports. For Secretary of State cases, Cloudgate Healthcare also prepares response letters and other required clinical narratives.
RISK CLASSIFICATIONS AND INTERVENTIONS
Many clients contact our office with questions about what it means to complete an “intervention.” The term "intervention" refers to the educational or treatment programming ordered by the court or required for a Secretary of State hearing.
A clinical intervention for legal purposes is not the same as an informal or family-led intervention. In legal and administrative settings, intervention is a required clinical response based on risk level. The following list describes the recommended interventions for each risk classification:

Minimum Risk: No evidence of problematic substance use. Requires completion of 10-hour DUI Risk Education course.

Moderate Risk: Evidence of use, but no diagnosis of a substance use disorder. Requires 10-hour Risk Education and 12 hours of early intervention.

Significant Risk: History of a prior DUI or problematic use. Requires 10-hour Risk Education, 20 hours of outpatient treatment, and continuing care.

High Risk: Indicators of dependency or multiple prior DUI offenses. Requires 75 hours of outpatient treatment and continuing care.
Our facility is licensed to complete both in-person and online DUI risk education, virtual DUI treatment (all risk levels), and online evaluations for Secretary of State hearings. We can also complete evaluations for court in many service areas outside of Cook County.
ILLINOIS SECRETARY OF STATE LICENSE REINSTATEMENT SERVICES

Alcohol/Drug Evaluation for Secretary of State Hearing: A comprehensive clinical interview and review of records to assess substance use and establish risk classification.

Detailed Chronological Alcohol and Drug History: A written narrative documenting the individual's alcohol and drug use over time.

Treatment Needs Assessment: A clinical review to determine whether further treatment is required before eligibility for driving relief.

Updated Evaluation: Required when a previous evaluation is outdated or after a denial.

Investigative Report: A written document addressing the nature and circumstances of any prior offenses, substance use history, and related factors.

DUI Risk Education: A 10-hour education course required for minimum, moderate, and significant risk classifications.

Response to Denial: A clinical letter addressing the reasons for a denial at a previous hearing.

Letter Ruling Out Dependency: When clinically appropriate, a letter stating the client does not meet criteria for dependency. The letter must explain how dependency was ruled out by the provider.
All driver’s license hearing documents are drafted in compliance with Illinois Secretary of State administrative code, specifically 92 Ill. Adm. Code 1001.90 and 1001.440.
OUT-OF-STATE RESIDENTS AND ILLINOIS DRIVER’S LICENSE HOLDS
Cloudgate Healthcare regularly assists out-of-state residents who need to complete a drug/alcohol evaluation or DUI treatment with an Illinois-licensed provider. This situation typically arises when a client has an Illinois hold that prevents them from obtaining a driver’s license in their state of residence. Our agency is SUPR-licensed to complete outpatient clinical services for Illinois license reinstatement hearings, including:
