Cloudgate Healthcare is an approved DUI treatment provider for individuals with a DUI case in Dupage County, Illinois. Our facility is authorized by the court to provide the following services for Dupage County DUI defendants: Minimum, Moderate, Significant, and High Risk interventions, including substance use treatment and/or DUI risk education.
We are also licensed by SUPR to provide drug/alcohol evaluations, DUI risk education, and drug/alcohol treatment for driver’s license reinstatement hearings with the Illinois Secretary of State. Our agency understands the drafting requirements for submission of clinical documents at driver's license reinstatement hearings. DUPAGE COUNTY COURT REQUIREMENTS AND APPROVED PROVIDER STATUS

DuPage County DUI defendants are required to complete their DUI evaluation through the court's designated evaluation agency. This initial evaluation must be completed prior to sentencing and is used by the court to determine appropriate recommendation for education and/or treatment.

While the court controls the evaluation provider in Dupage County, individuals are permitted to choose an approved agency on the Dupage Provider List for Risk Education and substance use treatment services. Cloudgate Healthcare is an approved provider for DuPage County. As an approved provider, our agency is authorized to provide the following services for individuals completing a DUI sentence for the Dupage court:

THE CLOUDGATE PROCESS FOR DUPAGE COUNTY

1. Initial Contact
Clients begin by contacting Cloudgate Healthcare to ask questions about our services. If the client chooses to work with our facility, we schedule an intake appointment. Appointments are available during daytime, evening, and weekend hours to accommodate work, school, or family commitments.
2. Evaluation

Dupage Court Cases. After obtaining an evaluation with the Dupage court provider, the client provides our agency with the report for review. The court evaluation informs our agency of the client’s risk classification, as well as the minimum required intervention to satisfy the client’s sentence. A client may be classified in their court evaluation as minimum, moderate, significant, or high risk. Our agency is licensed to perform all levels of intervention in the outpatient setting.

Secretary of State Hearings. In cases where a Dupage resident contacts our office to attend a driver’s license hearing, our agency is licensed to complete the drug/alcohol evaluation for the Illinois Secretary of State. The first clinical step involves completing an evaluation with our office to determine the appropriate risk classification. During the evaluation, we also review the client’s current court purposes driving abstract, as well as any sworn reports from DUI arrest(s).
3. Scheduling of Required Services
Based on the risk level determined during the evaluation, clients can schedule with our facility to begin their applicable intervention. This can include risk education, early intervention services, outpatient treatment, or a combination of services. The recommended intervention varies depending on risk classification. Cloudgate Healthcare provides these services in accordance with state guidelines for minimum, moderate, significant, and high risk classifications.
4. Ongoing Clinical Support
Throughout the education or treatment process, clients receive professional support tailored to their needs. Treatment includes identifying reasons for substance use, prevention strategies, as well as discussion of potential legal and health consequences of substance use. We provide clinical progress updates and discharge planning. High Risk and Significant Risk clients also receive assistance with scheduling continuing care, when warranted.
5. Document Preparation and Delivery
Once services are completed, Cloudgate Healthcare provides the client with documentation. This may include the drug/alcohol evaluation, treatment documents, DUI risk education certificate, and continuing care reports. If a response to denial is needed for the Secretary of State, we also address any outstanding issues in a written letter for submission to the Secretary of State. Documents are delivered to the client for review by the court or the Illinois Secretary of State.

SUPR LICENSING AND ILLINOIS LICENSE REINSTATEMENT HEARINGS
The Illinois Secretary of State permits any SUPR-licensed provider to complete evaluations, education, and treatment services for license reinstatement hearings. Cloudgate Healthcare is fully licensed and qualified to assist with the full range of Secretary of State hearing services.
